We may use and disclose your information for treatment, payment, and healthcare operations, and as otherwise permitted or required by law.

Your Rights

You have rights regarding your health information, including the right to access your records, request corrections, request restrictions, request confidential communications, and receive an accounting of certain disclosures.

Our Responsibilities

We are required to maintain the privacy and security of your protected health information and to notify you following a breach of unsecured information as required by law.
